Treatment options for avascular necrosis include conservative management, joint replacement, core decompression (to relieve pressure in the bone), and osteotomy (cutting and reshaping of the bone). Conservative management involves bed rest, partial weight bearing with crutches, weight bearing as tolerated, and the use of nonsteroidal anti-inflammatory agents for pain management. This approach generally does not help stop the progression of disease. Joint replacement, such as total hip replacement for a patient with avascular necrosis of the femoral head, has been used with mixed results. Some patients who undergo joint replacement experience poorer outcomes compared with patients who have joint replacement for other reasons.
